RESPONSIBLE TO: CARE COORDINATOR SUPERVISOR
Position Summary: Under the direction of the Case Management Supervisor, the care coordinator is responsible for assessments, case plan development, authorization of service, referrals, and maintenance of case records.
RESPONSIBILITIES AND DUTIES INCLUDE:
1. Administration of the appropriate intake form, including a comprehensive needs assessment
2. Development of case plan
3. Making appropriate referrals and responding to applicant/client requests
4. Authorization of services; and
5. Maintain case records, including documentation of follow-up, reassessment, and termination
6. Maintain community resource manual
7. Maintain effective working relationships with provider agencies
8. Travel as assigned
9. Attendance at training as required by program standards
10. Retain responsibility for all case aide activities related to case management
11. Maintain case management duties such as preparing reports per policy and procedures of IDOA, quarterly calls and follow up to participants as needed
12. Follow all policy and procedures as outlined by IDOA as well as Mature Solutions
MINIMUM QUALIFICATIONS:
1. Be an RN, or have a BSN, or have a BA/BS degree in social science, social work or related field. One year of program experience, which is defined as assessment, provision, and/or authorization of formal services for the elderly, may replace one year of college education up to and including four years of experience replacing a
baccalaureate degree; or
2. Be a LPN with one year of program experience which is defined as assessment and provision of formal services for the elderly and/or authorizing service provision.
3. Be waived for persons hired/serving in this capacity prior to December 13, 1991.
